NorrFam Posted June 30, 2007 Posted June 30, 2007 You should be able to fill most of it yourself. Unless you need the tetnus date or something. Then just bring it with you. Its collected at tech. Its only in case they need it cause you get hurt and cant tell the emergency folks the answers at the time. Quote

The procedure is to fill out the statement of your med history, the have the doc cert and do all the tests, then give it to your reg person or forward it to Dave Ho, NASA NorCal. Just completed all that stuff and it was handled well. If you are an HPDE student, you don't need it. However, it shows really good understanding of the risks involved and would be welcome if you ever had a mishap. It seems you are heading for racing maybe? Quote
Renntag Posted July 2, 2007 Posted July 2, 2007 as stated above, the required med form is only for racers....as stated at NASAproRacing.com It is always a good idea, however to have the same info available even if not required. Motorcyclists and other extreme sports participants will put medical info on a sticker on their helmet. you could also print up a card and where it around your neck like a back stage pass or something. Anything you can do to provide info will guarantee you better medical service and thus decrease the club's liability in the long run. Quote
