How To Complete Your Medical Application

The NASA Medical Application process is required for ALL NASA Members who are participating in Competition Racing. This application must be completed following your NASA Competition License Application Upon completion of your form, you will receive an automated email containing the summary of your application along with an additional Medical Evaluation Form to be completed by your Personal Physician or Medical Examiner. Once all of the required documents have been uploaded, NASA Staff will review and let you know if there are any missing, incomplete or additional documents needed. Once the application has been approved by the NASA Medical Director, Your Regional Director will process your Competition License and notify you.

1) Read and acknowledge the requirements for the NASA Medical Application Process

 

2) Fill out completely the required information

 

3) Upon submission, you will receive an email with a PDF form of your application along with a Evaluation form that you will take to your physician for completion. This email will also contain a unique link assigned to you so that you can upload the required documents. DO NOT delete this email or share the link with anyone else. 

 

4) Print and take the form to your doctor to have an evaluation performed. Ensure that all required information is filled out completely and that all pages that require signatures are completed.

 

5) Upload the required documents using the link from your email. You will have an option to select faster processing times if you have an upcoming race event that falls outside of the normal Medical Application processing times.
