Printable membership & event forms? Where?

I have a friend who doesn't have access to the Internet(yes, he's the one). I was trying to find a way to print out membership application & event registration forms, but couldn't find what I needed. Are these available? If yes, then where, & if not, then what would be the proceedure for non-online dealings?

It appears that the form was pulled down. Have him call 510-232-6272 and they will take the information over the phone or fax him a form.

 

We will put the printable form back up next week.
