Keith Posted January 12, 2006 Share Posted January 12, 2006 I am certainly not an expert, but there are specific forms that need to be signed by the physician and sent along with your license app. I have to assume that those specific responses on the form need to be at Nationals on the app which would lead me to conclude that you would at least need your doctor to fill out the NASA-specific form.
